Newshub can reveal two people have mistakenly been given expired vaccines.
One person received an expired dose in Auckland and another in Wellington in what the Ministry of Health is calling "human error".
As a result, immunisation experts have issued a reminder to vaccinators.
"An IMAC [Immunisation Advisory Centre] reminder has also recently gone out to vaccinators to ensure they are checking expiry dates at time of receipt and ensuring vaccine expiry date is checked with vaccine preparation," a ministry spokesperson told Newshub.
The blunder comes after it was revealed five Aucklanders who turned up for their COVID-19 vaccine in July may have got a dose of saline solution instead.
The mistake happened at the Highbrook vaccination centre in Auckland and the problem was discovered at the end of the day, when staff realised there was an extra vaccine vial left over.
Another six Christchurch residents were given a "very low dose" of vaccine by mistake at the Wigram vaccination clinic, where vaccine stock did not match the number of doses administered.
Earlier this week, Stuff revealed a man was vaccinated against COVID-19 up to 10 times in one day on behalf of other people. The Ministry of Health has since launched an investigation.
