A report has revealed a chain of agency failings which allowed paedophile and killer Phillip Smith to escape to Brazil.
The high-level inquiry found that it wouldn't have happened if someone had picked up the phone and done some basic checks.
National MP Judith Collins and Labour deputy leader Annette King joined the Paul Henry programme today to talk about the issue.
