Belgium international Eden Hazard scored twice in a man-of-the match performance in Chelsea's 4-1 Europa League final win over Arsenal, and it looks set to be his last match for the London club.
Hazard, who has been heavily linked with a move to Real Madrid, told BT Sports after his side's victory in Baku that it was likely his last game for the Blues.
"We will decide in a few days and the only target in my mind was to win this final," Hazard said. "I have made my decision already and now I'm waiting on both clubs. I think it is a goodbye, but in football you never know.
"My dream was to play in the Premier League and I have done that for one of the biggest clubs so maybe now it is the time for a new challenge."
Hazard, who joined Chelsea in 2012, provided the pass for Pedro to score Chelsea's second goal after Olivier Giroud opened the scoring.
He then scored twice himself - firstly from the penalty spot to make it 3-0 and his side's fourth after Alex Iwobi had pulled one back for Arsenal.
Hazard has made 352 appearances for Chelsea since joining from Lille, scoring 110 goals and winning two English Premier League titles, an FA Cup, a League Cup and two Europa League titles.
